This is not an insurance plan, it is a discount savings plan which means you enroll here and pay here. You can join the plan and start saving immediately, it's that easy!

Individual & Spouse - $549.00
Family Plan (two adults & two children under 14) - $849.00
Additional Children (up to age 14) - $199.00
Additional Family Members (over age 14) - $249.00

Examination and cleaning (2x in contract year) - Free
Annual x-rays (1x in contract year) - Free
Fluoride treatment (2x in contract year for children under 14) - Free
Additional x-rays - 20% off
Sealants - 20% off
All restorative treatment (excluding cosmetic) - 20% off
All periodontal treatment - 20% off
Implant related treatment - 10% off
Orthodontics - 10% off
Cosmetic dentistry - 10% off
Annual fee is due at time of enrollment and is in effect for 12 months from that date.
